Brief Paperwork

Upon entering the office, we have some brief paperwork for you to fill out. The paperwork provides us with general information about yourself and your condition. For your convenience, you can find the forms on this website. To save time, you may choose to download, print, and complete those necessary forms prior to your appointment. If you have any questions, please do not hesitate to call the office.

Consultation History Examination

Dr. Hokokian will consult you to learn about your current complaints then ask you questions about your history of injuries and health concerns. He will then perform a comprehensive orthopedic and neurological examination to learn the extent of your complaints and/or injuries. At times, he will learn that your condition is one that requires a referral to another specialist or facility.

X-ray Studies

Your specific condition may require Dr. Hokokan to take x-rays in our office or request that they be taken at an imaging center or hospital. This is to rule out more serious conditions and assist him in developing the most effective treatment plan for you and your condition. You may also be referred for other special studies such as: MRI (Magnetic Image Imaging), CT (Computer Tomography) or laboratory tests (CBC - complete blood count) or UA - (urinary analysis).

Treatment

After your consultation and examination have been completed and all the appropriate imaging or tests have been acquired, you will be scheduled to have these findings reviewed. Treatment generally will follow this review. This may include chiropractic manipulation, physio-therapies, and/or soft tissue massage.

Home Instructions

Prior to leaving, patients will be given instructions regarding limiting, changing, or continuing their daily activities or to be placed on total or partial disability from work. You may be prescribed and instructed in the use or application of ice packs or moist heat as well as prescribed exercises or stretches to be performed at home.

Schedule Your Next Appointment

Generally, new patients and returning patients are able to be scheduled within 1-2 days of calling (not including office closure for holidays and weekends). Treatment options for your condition will also be discussed during this time.
